Sergio Santos vs. John Farrell - 16th inning

So not too much has been made of this, but what happened in the bottom of the 16th? I can't seem to find much info on this. It was just another odd situation that occurred in extra innings that makes you wonder (ie. davis' botched bunt and Arencibias missed sign). This Instance, however, seems to have been lost in all the hoopla in all the game recaps.
One of our prized acquisitions was all set up to close the game and had to embarrassingly jog back into the bullpen. Sergio seemed flustered, and not surprisingly walked his first batter I believe, and Farrell seemed a little perturbed at what transpired. He was out quickly to rectify the situation after Perez got the first out, but my question is what do you think happened?
Everyone and their grandmother thought that Santos was coming in for the bottom half, and considering the fact that Farrell openly admitted to mishandling the bullpen and specifically the closer situation last year, I'd say it wasn't the best of starts with out new closer. My guess is that he wanted to see if Perez could make an out or two, as he didn't want to bring in his final arm unless they got a baserunner.
Nevertheless, I pesonally think that since he was staked a 3 run lead, you let your big arm go out there and show us why we got him. Thankfully Sergio closed it off after an awkard situation and hopefully this was one of the first things addressed after the game.
So what happened? Does anyone know? I assume it's poor communication between the bench and the bullpen. Has this been addressed in the media at all? Farrell obviously knew what he wanted to to, but the 'pen seemed unaware.
As I said, thankfully it all worked out, and the media hasn't blown it out of proportion, but WOW, did that seem awkward and hopefully it's water under the bridge.
