Simulated Games

When it says that a pitcher is going to pitch in a simulated game before he begins pitching in the minors to come back, what does that mean? I know it means he will pitch in a game that is pointless but who plays in it and when does it happen and where? Just always wondered what simulated games were about and never really cared enough to ask until now.
