Cedric Ceballos also had 40 on 14/15 shooting on March 9, 1993 (93.3%).

But probably in earlier seasons Wilt was even better. I don't know about any of his games with +40 pts and +90 FG%, but for example he had insane January in 1962 when he scored 73 points against Chicago and had 60.4 FG%. Next day he scored 62 against mighty Celtics with Bill Russell and had 60 FG%. At the end of that month, again against Celtics, he had 54 points on 75 FG%!
