Insurance exemption list? Who's on it?

Hi guys,

I was wondering if you could help. From bits and pieces I have heard I believe that over the last few years that there are 12 players the NBA insurance company can exempt for whatever reason (mainly cost and intrenational ball during Summer)

Luol Deng is on the list
Ilgauskas used to be

I think the Amare USA dropout was due to this too.

Can anyone confirm the names of the players and the overall maximum number?
Also does it mean that players can be changed during the contract ie Dunleavy can take the place of Ilgauskas or is it like the rules of 'shotgun' you gotta call from the beginning as early as possible?
