I only get notifications added to the drop down when I should - that is working correctly. The problem is the notification counter. It will sometimes fluctuate while I'm on the site as I go from screen to screen - maybe telling me I have 3 new while it just indicated 1 earlier - yet when I click on it, they are just the same notifications.

This used to work perfectly, but the past several weeks the counter seems to have gone a little haywire.

As a follow up, I noticed the number indicator on notifications growing as I was just surfing the site. I went to the drop down and just saw the same old notifications - nothing new. Yet I noticed I had some new quotes of my posts in some threads that weren't in my drop down list.

I think the counter is picking up quotes and "and-ones" as they happen, but the drop down box is lagging and it can take a while to get there.

Right now it keeps telling me I have one notification. I left the site, came back, and it's still saying one notification. But my last one in the drop down is 3 hours old.

Look at your complete list of notifications and see if you missed reading one:
