When his entire self identity and worth is built around being able to physically dominate his peers, Zion has to risk losing the "superstar tag" if after losing weight he cannot dominate. One could infer that he doesn't believe he has the enough fundamental skills, athleticism and IQ to replace the size advantage he can count on even at the risk of his health and longevity.
Ben Simmons faced the same personal challenge. As long as Ben was allowed to dominate how he's always dominated while avoiding shooting situations, he never had to risk the "good but not great" label and the loss of superstardom.
For Ben, those chickens came home to roost and he'll now always be known as a good but flawed player. The clock is ticking on Zion.
