I actually remember reading that the teams petition the league for new uniforms and the league comes up with the designs. Then the team picks out of the designs.

^ah...that would be wack...but that's certain proof that the "shoe companies" arent making them
on another note...whatever guy on the league's payroll that came up with that twolves design should be fired...taken out back...shot in each pinky toe...then stripped naked...and made to walk the streets from new york to minnesotta with a sign that says "it was me" covering his jock

I believe its that the in house design team or someone the organization hires, designs a few concepts and it goes back and forth until they compromise. Some teams will be denied certain colors because the colors are being over used. This process takes about two years, same as in the MLB (I know cos I work for an MLB team as a graphic designer)
