we also pay among the highest rates for legal, for education by a large margin, for TV's, top 5 in real estate (tokyo, london among a few others are higher). we pay more for concerts, sporting events, internet service, for a tow truck, for food, for drinks at a trendy bar. and on and on and on.

yes canada is equally expensive. as is anything in london, tokyo and at least 10 other places in the world. capitalism, free market, even corruption? poor policy? perhaps just the cost of doing business in an advanced society? call it whatever you want. they all play a role to some degree.
the point is that one way or another americans pay a lot for nearly anything and everything.
