Let's think about this - George Hill is probably the much bigger of the long-shot because of how many good point guards there are in the East (Rajon Rondo, Deron Williams, Jrue Holiday), but I think David West has a legitimate shot to be named by the coaches.
Chris Bosh, Carmelo Anthony and LeBron James will probably be the voted-in front-court by the fans, and that's fine. Amar'e Stoudemire won't be named by the coaches since he will have barely played by the ASG, Kevin Garnett will probably be there one way or another, but after that, who? West, to me, should and probably will trump Carlos Boozer, Al Horford, and Josh Smith.
