As Suggs is unlikely to pass a physical right now, I suspect you don't have to worry.

Hes expected to make a full recovery. INjury concerns isn't the reason orlando isn't doing this trade. Garland also has been ICU level health this season.

It really comes down to whether or not they plan to resign Ty Jerome and Sam Merrill. If they let them walk, then yeah I see no reason why they would need to make any drastic roster changes. They'll have a decent luxury tax bill and will only be slightly over the 2nd apron, which is manageable.
If they plan to resign them then that potentially changes things. Assuming Jerome gets somewhere close to the $14.3M max that Cleveland can offer him, and assuming no other salary cost cutting measures are done, that will cost them near $80M in additional luxury tax. Anything they give Merrill obviously pushes that number way up. As an example - if they give Jerome $14.3M and Merill $6M, that $6M will cost them another $38M in luxury tax. In this scenario they'd be looking at $175M in luxury tax. I know Gilbert is rich and doesn't mind paying luxury tax, but that seems exorbitant.

Seems like in this scenario they could look to dump Okoro and/or Wade into some team's MLE at the cost of some 2nds and cash, which would dramatically reduce their LT bill (although it probably wouldn't get them under the second apron)
Just looking at your math in reverse, dumping Wade into some team's MLE would save them about $45M in salary and LT payments.
