I would hope that Doc Rivers would name the next highest-voted Forward into the starting lineup first (Chris Bosh), and that David Stern would name the replacement in reaction.
Who better to replace Kevin Garnett than Chris Bosh, who has virtually the exact same skill-set?
Either a perimeter-oriented and/or undersized center would need to be named to the squad to replace Bosh's role with the second unit. Rasheed Wallace (both), Emeka Okafor (*undersized*), Zydrunas Ilgauskus (perimeter) and Andrew Bogut (neither, but 14/7 is equivalent to Rasheed's production) would be candidates.
I would have it look like:
Kidd/Billups
Wade/Pierce/Hamilton/Johnson
James/Butler
Bosh/Jamison/Garnett*
Howard/Wallace
This wouldn't really mess with the dynamic of the lineups, and a reasonable *snub* (one of the better centers on the second best team in the East) would get the honor.

Rasheed Wallace, Andrew Bogut, Emeka Okafor, Shaquille O'Neal (if you wanted to get technical, I believe he could play for the East), and *perhaps* Samuel Dalembert. Only because he's posting a double-double, shooting 50%, and is a shot-blocker. and is super-athletic and therefore EXCITING.

Doc will name a replacement in the starting lineup and Rasheed gets the spot deservedly so since a back-up Center was needed. If Butler sits it out Hedo will most likely get the spot.
