My vote has to go to the '03 Dallas Mavericks against the Portland Trailblazers. They were up 3-0 and were trailing in the fourth quarter in Game 7 on their homecourt.

Dwight Howard on his FT struggles:

Its not the worst one, but the lakers avoided blowing a 3-1 lead in the conference finals to the blazers.

05 playoffs Wizards @ Bulls

Wiz blow a 10 point lead with less than 40 sec. to go in the game, but gil wins the game at the buzzer anyways.

How can something qualify as a 'choke job' if the team wins?

Orlando Magic up 3-1 against the Pistons then losing in 7 (the McGrady "feels great to make it to the second round" series)

The Pistons losing four straight in the ECF last year after being up 2-0
