It is not unusual for a person not wearing boxers to have their penis slip out of their shorts, especially when sexually excited. Look at the guy's hand and his facial expression. He's grabbing the other guys balls pretty much, and he looks excited about it.

After reading the question, Joey Graham was the first to pop in my head, then Karl Malone.

He isn't that ripped compared to others.
