I don't know what happened in this case. But, I've had many friends struggle with drug abuse, and one of the things I learned in trying to support their recovery is that recovering drug addicts are especially vulnerable to OD-ing because they have a reduced tolerance to their drug of choice and tend to overdose upon relapsing. I lost a friend a couple months ago who OD'ed after being clean for several months.
