Despite my uncle's lack of recollection, I think we can say that he definitely brought this sheet of paper to two separate games in different seasons. There's too many guys that weren't one each others' teams (Gary Gregor, Dave Lattin, Bob Warlick, George Wilson, Lamar Green, Connie Hawkins, Neal Walk).
My uncle didn't miss very many players - Paul Silas, Art Harris, and Jerry Chalmers were the only guys he missed that got more than 1,000 minutes in a season. I'll have give him a high-five next time I see him.
At least one of those occasions - when Jerry Sloan and Barry Clemens signed - can be dated. Both played for with Chicago in '68-'69 (but did not play together in '69-'70), so they probably signed it together at Veteran's Memorial on either Halloween, 1968, or Jan 24, 1969.
